Vital Bridges Food Program

Participants visit Vital Bridges’ grocery centers and select from an array of nutritious foods at no cost to the participant. There are three grocery centers in the Chicago neighborhoods on the north, south and west sides. Eligible participants can shop weekly selecting nutritious food equal to 10 meals per week.

Eligibility to the food program must return the documents on the application checklist including verification from a physician of an HIV diagnosis and an income of no more than $2,942 per monthly (excluding SNAP benefits). Eligibility is reviewed annually.

Nutrition services are available to anyone who is HIV+. You do not have to be receiving food services to see a Dietitian.

Referral / Intake

Referrals to the food and/or nutrition program can come from any agency. Please follow the application checklist for required documents.

Appointments / Walk-ins

After intake is completed, eligible participants participate in food & nutrition services during specific hours at approved site. The program does not accept walk-ins.


All services are free of charge.
