Vital Bridges Food Program

Participants visit Vital Bridges’ grocery centers and select from an array of nutritious foods at no cost to the participant. There are four grocery centers, three in the Chicago neighborhoods on the north, south and west sides and one in the suburb Elk Grove Village. Eligible participants can shop weekly selecting nutritious food equal to 10 meals per week. Suburban participants who do not have transportation to a site can receive home-delivered food boxes.

Participants must show verification from a physician of an HIV diagnosis and an income of no more than $1,471 per monthly (excluding SNAP benefits). Eligibility is reviewed annually.

People using these services must be HIV+ and in most cases living on incomes of $14,000 or less.

Appointments / Walk-ins

After intake is completed, eligible clients participate in services during specific hours or by appointment with a case manager, housing specialist or registered dietician. The program does not accept walk-ins.
