Vital Bridges Center on Chronic Care

Vital Bridges Center on Chronic Care helps people living with HIV/AIDS throughout metropolitan Chicago improve their health and build self-sufficiency by providing food, nutrition counseling and education, housing, case management and counseling services.

Services Offered
  • Food distribution through four grocery centers and home-delivered grocery boxes in the suburbs
  • Transitional and supportive housing focused on the west side of Chicago and the near western suburbs
  • Case management in partnership with the AIDS Foundation of Chicago
Eligibility Criteria
  • People using these services must be HIV+ and in most cases living on incomes of $14,000 or less
  • Housing programs have specific criteria based on funder, geography, and type of housing service

After intake is completed, eligible clients participate in services during specific hours or by appointment with a case manager, housing specialist, or registered dietitian.
