Vital Bridges’ case management team provides medical and case management services to people living with HIV/AIDS (PLWHA). Case managers link participants to medical services pertaining to the treatment cascade for HIV. Case managers also provide linkage to psychosocial services, food/nutrition services and other related services that would address the participant’s current deficits.
