Vital Bridges Center on Chronic Care’s case management team provides medical and supportive case management services to people living with HIV/AIDS (PLWHA). Case managers link participant sot medical services pertaining to the treatment cascade for HIV. Case managers also provide linkage to psychosocial services, food/nutrition services and other related services that would address the participant’s current deficits.

After intake is completed, eligible clients participate in services during specific hours or by appointment with a case manager, housing specialist or registered dietitian. This program does not accept walk-ins.
