Vital Bridges’ case management team provides medical and case management services to people living with HIV/AIDS (PLWHA). Case managers link participants to medical services pertaining to the treatment cascade for HIV. Case managers also provide linkage to psychosocial services, food/nutrition services and other related services that would address the participant’s current deficits.

Referral is from the AIDS Foundation of Chicago, please call 312-922-2322. We also accept walk-ins. Upon intake documents such as proof of HIV diagnosis, income, labs, and insurance are required.
Referral from AFC preferred but walk-in ok. Application w/ith required documents reviewed at time of intake.

After intake is completed, eligible participants participate in services by appointment with a case manager.
All services are free of charge.
Appointments with case manager are required. Monday through Friday 9am – 5pm.
