Heartland Health Outreach Oral Health offers complete oral health care services. Staff provides regular dental checkups and cleanings as well as fillings, extractions, and other services.

Eligibility

Services are available to people who are experiencing homelessness, and have already attended a medical visit with a primary care provider at Heartland Health Center, or received a referral from a Heartland Health Center medical provider. For those individual living with HIV/AIDS, they must have attended a medical visit with a primary care provider (either within HHO or outside), provided laboratory tests (CD$, viral load) at leas twice yearly, and obtained a referral from a medical provider.

People who are experiencing homelessness must have a medical visit with a primary care provider at Heartland Health Center – Uptown; People who are living with HIV/AIDS must have a medical visit with a primary care provider (this may be a Heartland Health Center – Uptown Provider, or it may be a provider from another health center), laboratory test (CD4, viral load) updated at least twice yearly and a referral updated at least annually from a primary care provider.

Referral / Intake

People who are experiencing homelessness must have a medical visit with a primary care provider at Heartland Health Center – Uptown

People who are living with HIV/AIDS must have a medical referral from a primary care provider (this may be a Heartland Health Center Uptown provider or it may be a medical provider from another health center ), current laboratory tests (CD4, viral load).

Photo identification

If employed, proof of income (pay stub, official W2 from employer), or other income.

If participant has insurance, relevant insurance information (Medicaid card, Medicare card, private insurance card)

If eligible, Social Security Card

This program takes appointments. Appointments are required for service.
