HHO provides outreach and engagement services to individuals who are chronically homeless and who have been identified as the most vulnerable. The Vulnerability Index is used to identify these individuals based on the presence of live diseases, end-stage renal disease, HIV+/AIDS, age over 60 years, history of cold weather injury, three or more emergency room visits in the previous 3 months, 3 or more emergency room or inpatient episodes in the past year, and tri-morbidity.
