Benefits and Entitlements

The Benefits and Entitlement team assists participants with navigating public benefits. The program assists individuals with Medicaid, ACA Marketplace, SSI/SSDI, SNAP, and Cash assistance applications. We can assist with Medicare part D plan and Medicare Savings Program. The B&E specialists also help with referrals to disability lawyers and provide education on how to navigate the healthcare system.

Eligibility

Individuals must be receiving services from a Heartland Alliance Health program. Individuals can receive insurance enrollment assistance at Uptown site M-F from 9AM-4PM and at James West site Mondays, 9AM-4PM.

ACA Medicaid: age 19-64, citizen or qualified legal resident, income at or below $1366 monthly; SPD/AABD: disabled based on SSA criteria; Marketplace: qualified legal resident or citizen, income above $1366 monthly, and ineligible for Medicare or affordable coverage through employment; SNAP: citizen or qualified legal resident; income below $1619 monthly; SSI/SSDI: 65 yrs. of age and older or meet disability SSA disability listing, & citizen or qualified legal resident; Medicare: 65 yrs. of age and older or receive at least two years’ worth of SSDI payments.
