Assertive Community Treatment Team

HHO’s ACT team is an interdisciplinary team that has service availability seven days a week and is prepared to carry out a full range of treatment functions.  Participants should expect to be engaged no less than three times a week and as many as 7 times a week

Comprehensive assessment, individualized treatment planning, psychiatric services, nursing services, mental health counseling and therapy, group counseling and therapy, transportation (when appropriate), crisis intervention, case mangement, representative payee services.


Individuals must be eighteen or older, must be diagnosed with a serious mental illness, and may have a co-occurring substance use disorder but it is not necessary.

Referral / Intake

An individual is referred to the ACT team service when it has been determined that his/her needs are such that they need intensive outreach assistance in order to maintain in the community. By referral through integrated intake, 312-520-7267 or through engagement ont eh PATH team.
