Kimberly Bartolomucci: Kim serves as the Program Manager. She comes with years of non-profit experience and specializes in fundraising and event planning. Meesha Awan MPH: Meesha serves as the Education and Communications Specialist. She has five years of experience in public health and education. She works closely with the Oral Health Education team coordinating oral health outreach and trainings. She also disseminates oral health promotion messages and news through various platforms. Daniela Sanchez, BS: Daniela serves as the OHF Community Liaison/Case Manager. Prior experience includes working with unaccompanied minors providing workshops to help make their transition in the United States easier. At the Forum, she builds relationships with community partners and connects families with oral health services. Brenda Velazquez, BA: Brenda serves as OHF’s Case Manager and comes to us with a Bachelor’s Degree in Sociology. She has an extensive case management background working closely with families and children experiencing domestic violence. As a case manager she links families with the dental services they need. Kianna Walton BS: Kianna is our Program Assistant. She recently graduated from the University of Illinois at Urbana-Champaign, where she received her Bachelors in Community Health. Prior to joining OHF, Kianna participated and facilitated several health education outreach programs at U of I. Kianna hopes to expand her knowledge of oral health while working at the OHF. Lusina Hernandez, BA: Lusina joined OHF in March 2016 as a bilingual oral health case manager and educator. She graduated with her bachelor of arts in psychology and prior to joining us, worked as a child advocate at CASA of Cook County, where she excelled in conducting interviews to ensure client’s needs where met. In addition to family outreach skills, she relies on her strong communication, interpersonal, and problem solving abilities to ensure the well-being of her clients. She is passionate about promoting health and helping families get the care and resources they need. Leilah Denise Odeh, MSW: Leilah joined the OHF team in January of 2017, serving as our Grant Writer and Business Development Manager. She holds a Master of Social Work from the University of Illinois and completed her internship with Heartland Health Outreach. Leilah returned to Heartland with extensive experience in serving low-income households and communities, specializing in program management, development, and evaluation. Within the forum, she provides oversight to the case management team, develops innovative strategies to enhance and create new OHF programs and increase visibility, and works directly with stakeholders to maintain strong relationships for all invested in the mission of OHF.
