Alejandra Valencia, DDS, MPH, MS: Alejandra serves as OHF’s Project Director. She worked as a dentist in public health settings for several years before specializing in dental public health. Her expertise is in community-based research, data analysis and interpretation, and health systems impact. She ensures that our work aligns with national initiatives, our data is accurate, and our model is strong. Phone: 773.491.2632



kimKimberly Bartolomucci: Kim serves as the Program Manager. She comes with years of non-profit experience and specializes in fundraising and event planning. She oversees and manages the oral health education program. She also coordinates all meeting related activities of the Forum and ensures that our project timelines are met. Phone: 312.636.3070



Leilah Denise Odeh, MSW: Leilah joined the OHF team in January of 2017, serving as our Manager of Program Development and Case Management Services. She holds a Master of Social Work from the University of Illinois and completed her internship with Heartland Alliance Health. Leilah returned to Heartland with extensive experience in serving low-income households and communities, specializing in program management, development, and evaluation. Within the forum, she provides oversight to the case management team, develops innovative strategies to enhance and create new OHF programs and increase visibility, and works directly with stakeholders to maintain strong relationships for all invested in the mission of OHF.

danielaDaniela Sanchez, BS: Daniela serves as the OHF Community Liaison/Case Manager. Prior experience includes working with unaccompanied minors providing workshops to help make their transition in the United States easier.  At the Forum, she builds relationships with community partners and connects families with oral health services. She also assists The Oral Health Education team by providing oral health education presentations in Spanish for the bilingual classrooms. Phone: 312.718.1663


brendaBrenda Velazquez, BA:  Brenda serves as OHF’s Case Manager and comes to us with a Bachelor’s Degree in Sociology.  She has an extensive case management background working closely with families and children experiencing domestic violence. As a case manager she links families with the dental services they need. She also assists The Oral Health Education Team by providing oral health education presentations in Spanish for the bilingual classrooms. Phone: 312.515.5030


Aaron Ziegemeier BA: Aaron serves as a community liaison and case manager at the forum. He comes to the forum with case management experience working with HIV/AIDS populations in St. Louis. He studied Political Science and Spanish at the University of Missouri-Saint Louis and Universidad de Belgano – Buenos Aires. He studied politics in order to better understand how different laws effect populations in different ways, and studied Spanish in order to work with the Spanish speaking immigrant populations. He is very passionate about human rights, travel and the environment.


Kianna Walton BS: Kianna is our Program Assistant.  She recently graduated from the University of Illinois at Urbana-Champaign, where she received her Bachelors in Community Health. Prior to joining OHF, Kianna participated and facilitated several health education outreach programs at U of I.  Kianna hopes to expand her knowledge of oral health while working at the OHF.


Tayler Stuger serves as a Community Liaison and Case Manager. She recently finished service as a Peace Corps volunteer and has five years of experience in education and community development. At the Forum, Tayler works with the OHF Education team in providing health trainings and bilingual services as well as with the case management staff to connect families to oral health services.






Mia Frances LaRocca, BS: Mia serves as an Oral Health Educator at the Forum. She recently graduated from Loyola University Chicago, where she received her Bachelor of Science in Anthropology and a minor in Spanish Language and Literature. During her time as a Social Justice Research Fellow at Loyola, Mia advocated for undocumented patients access to healthcare and developed an app to connect patients to health resources in Chicago.  Mia also comes to the Forum with experience working in dental anthropology at the Cook County Medical Examiners and the Field Museum of Natural History.  She is passionate about health literacy and she is looking forward to teaching with OHF!



Mona Van Kanegan, DDS, MS, MPH: Mona is the co-founder and a consultant to the Forum.  She has been a oral health provider in FQHC settings for almost twenty years, with extensive experience as a dental director and consultant to several community health centers. She now sits on the Executive Steering Committee as an advisor to OHF.
