Supportive Housing Services

The Supportive Housing Services program of Heartland Alliance provides case management to formerly homeless residents living in four Heartland Alliance subsidized apartment buildings. The social services program model is designed to assist residents in reaching housing stability and to promote personal growth and well-being. Services consist of case management, employment and career development, financial literacy programs, substance use counseling, and housing services. To qualify for Supportive Housing Services, an applicant must be homeless and meet the building entrance criteria.

Eligibility Criteria:
  • Must be homeless and able to submit homeless documentation
  • Must reside in Heartland Alliance building where services are provided 
   
Fees:
  • Monthly rent based on income
  • Market-rate apartments available
   
Applying for Services:

Services available only through applications at four Heartland Alliance sites.

   
Referral and Intake:
  • Requires housing application with Heartland Alliance and placement on waiting list if eligible
  • Social services assessment follows notice that applicant is being considered for housing
