Stable Futures

The Stable Futures program of Heartland Human Care Services moves individuals from area shelters to permanent housing. Participants lease apartments from private landlords, and Stable Futures provides a housing subsidy and intensive case management services for up to one year. Stable Futures supports achievement of self-sufficiency, interdependence and stability over a twelve month period. Stable Futures intensive case management model focuses on linking participants to income supports and social services in their communities so that they will have community support after program completion. Stable Futures houses 64 individuals at any point in time through two HUD grants. Stable Futures educates participants on tenant/landlord rights and advocacy, assists families in locating and securing housing that will be affordable after the Stable Futures program ends.  Stable Futures case managers and participants collaborate to develop skills in budgeting, housekeeping, problem identification, goal setting and achieving outcomes.

Eligibility Criteria:

Single homeless adults (minimum 18 years old).

   
Fees:

Monthly rent based on income.

We are located 1525 East 51st Street in the Village Food court. We are two blocks north of 53rd Street and three blocks south of 47th Street. We can be easily accessed by CTA and Metra rail stations.

Appointments outside normal business hours may be accommodated on an individual basis.
