Outpatient Mental Health and Addiction Services for People Who Are Homeless

Outpatient Mental Health and Addiction Services for People Who Are Homeless targets individuals who are living in shelters or on the street and who have a serious mental illness and, often, a co-occurring substance use disorder. We offer a continuum of services, most located in Uptown, that include outreach and engagement; Community Support teams that provide mental health treatment while helping individuals access housing and other supports; a day program that offers skill building groups, integrated dual disorder treatment, lunch, and social opportunities; psychiatric assessment and medication; and linkages to other supportive services.

Eligibility Criteria:
  • Must be 18 or older
  • Must be homeless
  • Must have a serious mental illness; may also have a substance use disorder

Sliding scale based on income.

Two blocks east of Lawrence CTA Red Line stop, or take the #81 Lawrence bus to Lawrence and Sheridan Road.

   
Hours:

Outpatient services staff are available from 8:30 a.m. to 5 p.m., Monday-Friday. 

The day program runs from 9 a.m. - 2 p.m. daily.

The Community Support teams spend 60 percent of their time off-site, meeting participants in the community.  Walk-ins are not permitted; see below for information on applying for services.

With advanced notice, Heartland Alliance services can be provided in more than 35 different languages.
