HIV Case Management Services

HIV Case Management Services offers medical services, supportive services, and in-home case management for adults living with HIV/AIDS. Case management helps participants develop life skills, live independently and adhere to their medical treatment plan. The program works to ensure participants are linked to primary HIV medical care and other health services; educates participants on maintaining medical adherence; encourages mental health and substance use treatment as indicated; assists participants in securing entitlements and benefits, appropriate and affordable housing, and other ancillary services. Referrals are only through the AIDS Foundation of Chicago.

  • Linkages to resources and services
   
Eligibility Criteria:
  • Must live within the Chicago Eligible Metropolitan Area (EMA) 
  • Must verify HIV positive status
  • Must have no more than $10,000 in cash/convertible assets for in-home case management
  • Must have proof of U.S. citizenship
  • Must have Physician Certification of Eligibility for in-home case management

This program takes appointments.
