Heartland Health Center – Lincoln Square provides affordable, high quality health care for babies, children, adults, and seniors living on the North Side of Chicago. We work to prevent illness and promote physical, mental, and social well-being by providing respectful services to all people, regardless of age, race, ethnicity, gender, language, sexual orientation, insurance status, or ability to pay. We also offer health care to immigrants and refugees and offer medical interpreters in many different languages.

  • Chronic disease management including diabetes, asthma, and hypertension
  • Medication discounts
  • Dental screenings for children up to age 5. (Dental services for older patients and for those requiring restorative care are available at Heartland Health Center - Senn.)
  • Mental health services
  • Free pregnancy tests
  • Nutrition counseling

  • Prenatal care – including one-on-one meetings with our prenatal case manager to help meet any special needs during your pregnancy and breast feeding support from our lactation specialist

Anyone is eligible for services.

   
Fees:

In order to ensure our services are affordable and accessible:

  • We accept Medicaid and Medicare
  • We accept some private insurance plans
  • We offer a sliding fee scale for individuals who do not have health insurance
  • We provide enrollment assistance for AllKids and FamilyCare
