Health Care for the Homeless

This program is located in the Chicago Uptown community at Heartland Health Center - Uptown and provides primary health care and oral health care  for people who are experiencing homelessness, including people who are sheltered but not in permanent housing.  This program also provides primary care services at outreach sites where people experiencing homelessness stay throughout the Chicago metropolitan area..   Additional services include case management, assistance with benefits and entitlements as well as referrals for mental health and/or substance use treatment or medical services not provided at the health center.



Services Offered:
  • Primary health care for men, women, and children who are experiencing homelessness
  • Oral health care for men, women, and children who are experiencing homelessness
  • Outreach to people who are homeless
  • Referrals for services (mental health and/or substance abuse treatment, or medical services not provided at the health center)
  • Case management services
  • Benefits and entitlements services

Must be homeless, according to federal definition.
