EPOCH (Every Positive Change)

The EPOCH Program is a supportive housing program for HIV positive, single individuals who currently have or have had a history of substance abuse issues. Participants are housed in scattered-site apartments mainly on the North Side of Chicago and are required to pay 30 percent of their income towards rent. EPOCH also provides case management, educational support groups, transportation assistance, and referrals to substance use treatment.

Eligibility Criteria:
  • Must have some income but be below 80 percent of area median income
  • Documented HIV/AIDS status
  • Current substance use or history of substance use
   
Fees:

Monthly rent is roughly 30 percent of income, based on HUD rent calculation.

Referral and Intake:
  • Requires tuberculosis test
  • Proof of HIV/AIDS status
  • Photo identification
  • Integrated Assessment (conducted by program once intake is initiated)
