A New Life In a New Homeland
“I wanted to finish school and live the American Dream,” says Chenge, an immigrant from Zimbabwe who fled his home country to escape torture. “There’s a lot of worry when you don’t know if you can stay in this safe place and stabilize your life.” Heartland Alliance helped him gain asylum, obtain a visa, prepare for and find a job, and integrate into American culture.

Heartland Centers for Oral Health provide dental health care for persons experiencing homelessness and/or persons who are living with HIV/AIDS. Our centers can provide general dental examinations and cleanings as well as fillings, limited extractions and other services.

Services Offered:

Eligibility Criteria:

• People who are experiencing homelessness must have a medical visit with a primary care provider at Heartland Health Center - Uptown
• People who are living with HIV/AIDS must have a medical visit with a primary care provider (this may be a Heartland Health Center – Uptown Provider, or it may be a medical provider from another health center), Laboratory test (CD4, viral load) updated at least twice yearly, and a referral updated at least annually, from a primary care provider.

Fees:

• Sliding Scale based on income
• Medicaid Accepted

Applying for Services:

All services are available by referral only by physician if HIV positive, or if you are experiencing homelessness.

Referral and Intake:

• People who are experiencing homelessness must have a medical visit with a primary care provider at Heartland Helath Center - Uptown
• People who are living with HIV/AIDS must have a medical referral from a primary care provider (this may be a Heartland Health Center – Uptown Provider, or it may be a medical provider from another health center), current Laboratory tests (CD4, viral load).
• Photo Identification
• If employed, proof of income (pay stub, official W-2 from employer), or other income
• If participant has insurance, relevant insurance information (Medicaid card; Medicare card; private insurance card)
• If eligible, Social Security Card
